Participant Application - Important, please read!

Per Yavapai County rules, each food vendor at Clarktoberfest 2022 must fill out the Health Department application which can be found online here: Yavapai County Health Department Application. Wineries and Breweries are not required to fill out a Yavapai County Health Department application.
